Tonio Fortebracci entered coma and was assisted by Judge De Santis. Tonio resumes shortly afterwards and De Santis decides to put him under guard. Ricky should be escorted to the hideout of Tonio by two cops, but Jason kills one of them and, disguising himself from this car on the car where Ricky and the other policeman Jason threatens him with a gun and shows where Tonio is hiding. Then he shoots him in the head.

Don Saro Ferlito speaks to Tonio of the assassination of Fortunato Di Venanzio, convinced he is the culprit, but Tonio assures him that he does not have that. The boy is beheaded in the stables of the riding school where he is riding and his severed head is put into the car of his father, who for the pain is shot in the head. Ricky disguised as a garter of the baker, handing over to Tripolina the head of Vito Veneziani and not just as Tripolina sees someone playing at the door.

Tonio enters armed in Tripoli's home and asks why he is killing the children of the maimed, but Erminia threw him behind and tied him. In the hospital at Sirenuse, the doctor reassures Fabiola and Angelica telling them that the baron will resume . While Tonio and Fabiola quarrel out of the room, Angelica unhooks the electrodes from her uncle's chest and puts them on her to prevent the machines from alarming the doctors, then closes the oxygen valve and kills the baron.

After the baron's death, U 'puppeteer makes it clear to Tonio that if he does not find a solution as soon as possible, he will have to get rid of him. After the cousin's funeral, Angelica forces Tonio to marry her threatening to tell her all about the game to the police and Saro Ferlito. After her wedding, Tonio leaves the church without even kissing her bride and back home drunk after many hours. Tripolina orders Paride to marry Santina, holding Erminia as a lover. The guy is forced to accept.

Tonio continues to treat coldly and contemptuously Angelica, trying to make her suffer; Meanwhile, the collaboration of the prosecutor Trapanese is gained. Tony shows his nephew tied up and gagged in the trunk, but he scares him by intimating him to release him. Tony responds to Billy's death by killing him. Then Tonio shoots Don Saro three gunshots on his chest, revenging his brother.
