New killer emerges out from nowhere, as he kills Frank's family. After Frank is depressed, he talks to Michael about getting revenge for him, but Michael says how he's done with it all, As their relationship collapses.
They name the new killer "The Mannequin" as he's making bodies into mannequins and then attaching bombs to them, and placing it front of victim's parents home. As Michael hears about this, he becomes overjoyed as he realizes something.
Michael gets up early in the morning to look for Frank but eventually finds him hanging from the rope. As Michael tries to solve everything together, he finds someone alive that he knows VERY well, as he can't believe his eyes.
Franklin Clinton and Michael De Santa reunite again, but this time Michael is suspicious about Franklin, because after all these years, he returned. Chase continues as police is chasing the Mannequin killer but finds absolutely nothing.
Michael finds the same cave he's been hiding 3 years ago. And as he explores it, he finds everything he has hidden outside the cave, burning down with Mannequin above it, as he knows exactly how this killer knows his past.
Michael realizes who is Mannequin as he starts to chase down him, but eventually finds him on the same bridge, that Chip was when Michael killed him. As Mannequin starts to talk about his past and how Michael always was a naive fool.
Another killer emerges, leaving Michael scared of being killed, as he leaves the city and gets addicted to drinking and drugs. Meanwhile Mannequin meets the new killer, and as New killer murder Mannequin, he leaves message for Michael.
New killer's name is revealed as "The Hook Man". As he delivers message to Michael, telling Michael how he's watching his every move and Michael can't get out from his hideout without seeing Hook Man. As Michael is tired of being scared.
Michael meets Hook Man under the bridge as they talk about what's going to happen in the future, as Hook Man offers Michael to turn himself in, while saying the word as how much moral of Hook Man will gain. Conversation turns a dark turn.
Michael doesn't turn himself in as he and Hook Man gets into a fight, as Hook Man overpowers him and says how Michael can't do nothing. Then he quickly tries to murder Michael, as Hook Man gets killed by Michael with secret gun.
Michael gets caught by Police as he doesn't have any choice more to turn himself in, as he gets into his new life as prisoner. Meanwhile someone in prison reveals a horrifying truth to Michael, leaving him considering his life in prison.
Truth is revealed as Michael's family is alive and is doing very well, as Michael can't believe his ears, and as Michael is confused, he meets them through phone call, as he's finally happy about it all, but he has questions for them.
