The pop star Skye Riley is preparing for a tour after one year in a rehab clinic. Skye had a car crash with her boyfriend Paul Hudson, who died in the accident, and now she goes to the Drew Barrymore Show to promote her comeback. Her manager and mother Elizabeth Riley and her assistant Joshuatake care of her to avoid a drug relapse. However, Skye feels pain on her back due to the accident and visits her drug dealer Lewis Fregoli to buy the pain killer Vincodin to help her. She finds Lewis totally insane, unpredictable and unstable, and out of the blue, he smiles and hits his face several times with a weight plate. One week after Rose Cotter's suicide,[a] ex-police officer Joel attempts to pass the Smile Entity's curse to a pair of criminals at their hideout. A shootout occurs, killing them both, which is witnessed by drug dealer Lewis. Joel flees the hideout and is fatally run over by a truck.
In New York City, successful pop star Skye Riley prepares for an upcoming comeback tour following a highly publicized struggle with substance abuse and a car crash that killed her boyfriend Paul Hudson. Under constant supervision from her mother and manager Elizabeth, along with their assistant Joshua, Skye sneaks out to visit Lewis after injuring her back during rehearsals. At Lewis' home, Skye discovers his erratic behavior when he sees something that Skye cannot see before he collapses, smiles at Skye and bludgeons himself to death with a weight. A traumatized Skye decides not to call the police, not wanting to be linked to Lewis' drug dealing.
Skye continues preparation for her tour but soon begins to have vivid hallucinations of smiling people as well as witnessing several inexplicable events. Music executive Darius invites Skye to a charity event he is hosting, which Elizabeth pressures her to accept. Skye also reconnects with her former best friend Gemma who she has not spoken to in a year after a falling out. That night, Skye has nightmares about Paul's death before an unknown number texts her, claiming to know that Skye is in danger. At the charity event, Skye sees "Paul" smiling at her and accidentally injures an elderly patron, humiliating herself in front of the record label.
Skye contacts the unknown number and meets a man at a bar. He reveals himself to be Morris, a nurse whose brother was also killed by the entity and has been tracking it. He explains how killing someone is the only way to pass the curse along. Morris also theorizes that the entity is a parasite that will die without a host, so he suggests to kill and resuscitate Skye to rid her of the curse, but she panics and flees. Skye is later attacked by the entity, who masquerades as several of her background dancers, which leads Skye to suffer a concussion. In a flashback, it is revealed that Skye had argued with Paul on the night of his death and that she intentionally caused the car to crash. Skye soon awakens in a wellness facility where she argues with Elizabeth over her behavior. She realizes that the entity is posing as Elizabeth who seemingly kills herself with a mirror shard. Skye attempts to flee but discovers that she is holding the shard.
Desperate to break the curse, Skye steals a gun from a security guard and leaves the facility. She meets Gemma, who drives her away. Skye contacts Morris, who sends her where to meet, before getting a call from Gemma, revealing that the entity has been taking her form. Skye takes control of the car and meets Morris at an abandoned pizzeria. As Morris prepares to stop Skye's heart in a freezer for eight minutes to be certain the entity is gone, Skye is attacked by the entity and, despite her attempts to kill herself, the entity overpowers and locks her in the freezer.
Skye then emerges on stage at her tour's opening night where she is confronted by a smiling version of herself, which bursts open to reveal the entity's true form. Resigned to her fate, Skye allows the entity to crawl inside her as she collapses. The now-possessed Skye gets back up and stabs herself to death with a microphone, passing the curse onto Darius, Joshua, a still-alive Elizabeth, and the rest of a horrified audience.
