Kabir is on a descending spiral of alcohol, drugs and one night stands. Kabir is a surgeon at a private hospital and is an expert at his job. Kabir is performing way too many surgeries, but he refuses to back off.Kabir is also working on his thesis.

In flashback, Kabir Rajveer Singh (Shahid Kapoor) is a house surgeon at Delhi Institute of Medical Sciences in Delhi, India. Despite being a brilliant student, he has severe anger management problems that earn the wrath of the dean of the college. Kabir's aggressive nature also earns him a reputation among his juniors as a college bully.

After having a brawl alongside his friend Kamal (Kunal Thakur) against members of the opposing team (from the Khalsa Institute of Medical Sciences) during an inter-college football match. The opposing team player deliberately provoked Kabir, and even punched him, but the referee red carded Kabir, leading to Kabir beating up the referee and the opposing player Amit (while chasing him all over the stadium and breaking his nose and face).

The dean (Adil Hussain) asks Kabir to either apologize or leave the college. Kabir says that he is loyal to the college, and he cannot keep quiet in face of blatant injustice. Kabir argues that accepting injustice becomes a habit in life and he does not want to turn into one of those people.Kabir initially chooses the latter but stays back after meeting first-year student Preeti Sikka (Kiara Advani).

Kabir and his friend Shiva (Soham Majumdar) enter a third-year classroom and announce that Kabir is in love with Preeti and asserts that she is exclusive to him. Kabir saves her from all the college ragging and kisses her on the cheeks the first time he meets her outside her hostel. Kabir decides who she can be friends with and also her roommates. He takes her for private classes with himself, instead of letting her attend her lectures.

On Holi, Kabir wanted to be the first one to apply color to Preeti, but Amit came to her hostel and assaulted her applying color to her chest and hips, knowing that she is Kabir's crush. Kabir goes to Amit's hostel and assaults him badly and warns him that he loves Preeti.

Initially afraid, Preeti starts adjusting herself to Kabir's overbearing attitude. She eventually reciprocates his feelings, and they develop an intimate relationship. Preeti cuts her feet on a shard of glass and Kabir shifts her to the boy's hostel. This is where they have sex for the first time and after that they have sex regularly. As the other boys' object to Kabir keeping a girl in their hostel, he shifts into a private apartment with Preeti, just outside of college.

Kabir graduates with an MBBS degree and leaves for Mussoorie to pursue a master's degree in orthopedic surgery. Over the course of three years, Kabir's and Preeti's relationship becomes stronger as Preeti visits him at his college. Months later, Kabir visits Preeti's house, where her father Harpal Sikka (Anurag Arora) sees them kissing and throws Kabir out.

Kabir's grandmother tells him that for Preeti, Harpal is her father, and she will not be able to marry him without her father's consent. Preeti convinces Kabir to come to her house again and seek Harpal's permission for her hand in marriage.

Preeti's father opposes her and Kabir's relationship since he dislikes Kabir's personality. Harpal continues to oppose their relationship, despite Kabir's attempts to explain their love. Kabir demands that Preeti must make a decision within six hours otherwise he will end their relationship. By the time she manages to visit Kabir's house, he is drunk, injects morphine into himself, and becomes unconscious for two days. Preeti is then forcibly married to someone from her caste. Kabir learns about the marriage from Shiva and goes to her house in protest. He is assaulted and gets arrested for making a scene. Kabir's father ostracizes him from the family home for damaging his reputation.

With Shiva's help, Kabir finds a rented apartment and joins a private hospital as a surgeon. To cope with his emotions, he starts taking drugs, attempts one-night stands, buys a pet dog and names it after Preeti and drinks alcohol; all of which are unsuccessful. Within months, he becomes a successful surgeon and a high-functioning alcoholic who is feared by the hospital's staff members, one of the reasons being his high surgery count. Kabir's self-destructing behavior and refusal to move on worries Shiva and Kamal. He persuades one of his patients, Jia Sharma (Nikita Dutta), a leading film star, to have a no-strings relationship with him, which he ends when she falls in love with him.

On a day off, Kabir unwillingly agrees to perform a life-saving surgery and collapses with dehydration. The hospital staff examine his blood samples, which show traces of alcohol and cocaine. The hospital chief files a case against Kabir, who accepts the truth on the grounds of violating his professional ethics during an in-house court hearing, despite Shiva making arrangements to bail him out. Kabir's medical license is canceled for five years and he is evicted from the flat. The next morning, Shiva manages to reach Kabir to convey his grandmother's death; he meets his father, and they reconcile. Kabir gives up his self-destructive habits soon after.

Some days later, Kabir spots a pregnant but dejected Preeti sitting in a park. He approaches and offers to raise the child with her if she is unhappy in her marriage. Initially silent, she bursts into angry tears, berates him for discarding her, and orders him to leave. Shiva then describes Kabir's obsession and self-destructive habits, which she was unaware of.

Shocked, she confesses that she left her husband three days after the marriage and has been working in a clinic to support herself. Overcome with anger, especially after hearing about his casual affairs, she had refused to contact him. She reveals that her marriage was never consummated and that the child is Kabir's. They marry and reunite with their families, with Harpal apologizing to them and the families coming together. Kabir and Preeti end up on a beach with their baby.
