Alana Hart, the owner of Four Horse Casino, is questioned by an investigative reporter regarding a murderer named "Bakwas" who reportedly works for her. Alana arranges for a cover-up, remains disposal, and a false alibi after the reporter signs a non-disclosure agreement and declines a $2 million offer.
Johnny and Clyde, serial killers and lovers, are shown in a montage committing indiscriminate crimes. When the stuttering attendant at the gas station wishes Johnny a safe trip to Rhode Island, Johnny takes offense, tortures him psychologically, and then shoots him for a candy bar and some cash. In an attempt to exact revenge, Randall Lock, a former sheriff who is overweight and alcoholic and whose daughter was tortured and killed by the two, hires bail bondsman One Time after learning that Johnny and Clyde have returned to Rhode Island.
Along with torturing a guard to get information about Alana's hidden cash room, Johnny and Clyde rob one of the casino's armored cars. When Lock and One Time ambush them, they use the guard as a human shield, and they manage to get away when insane killer Zhang launches a random attack to wreak havoc.
Johnny gathers his equally murderous foster-care brothers-the paranoid conspiracy theorist Candlestick, the suicidal thrill-seeker Butcher, and the insane arsonist Baker-to assist in the casino heist. They are warned by Candlestick that the location of the cash room was the site of a mass suicide by the demon Bakwas' satanic cult.
Lock tells Alana that the serial killers plan to rob her hidden cash room, and Guy, the casino's head of security, suspects them. After being provoked by this, Alana has the cult Elder perform a ritual that gives her the ability to control Bakwas and make him guard her casino using a mystical stone. Then Alana murders the Elder to keep the stone's secret.
Johnny and his team use illegal drugs in excess in order to prepare for the heist. After the perimeter guards are killed, Baker uses explosive charges to open the door and unintentionally kills himself. Lock and One Time, who are waiting in ambush, and Zhang, who is following them, soon encircle the crew as they enter the building. When Alana announces that the building has been locked and that everyone will be dead by sunrise, the Mexican standoff is broken up. One Time is killed by Bakwas.
A number of guards are killed as Johnny's crew descends toward the cash room in the basement. As Alana eliminates every potential witness, Guy realizes he has been betrayed. Bakwas kills Guy's elite team after they are trapped. Guy allows his team members to leave their posts. It turns out that Alana has already transferred all of the funds and is getting ready to move out of town and see her father again.
Using a bag of fake currency, Johnny and Clyde attempt to flee after Candlestick and Butcher are killed. Zhang removes them from the building as directed by Alana's father. Ambush-waiting once more, Lock murders Zhang and assaults Johnny. Lock confesses that Johnny murdered her father and covered it up when Clyde threatens to shoot her. Clyde pistol-whips Johnny and shoots Lock, but he describes his actions as romantic. Clyde pistol-whips Johnny and shoots Lock, but he describes his actions as romantic. Before Bakwas murders him, Lock, who is wearing a bulletproof vest, recovers to joyfully observe their corpses.
Guy is the only one who manages to get away, and he demands the stone from Alana while holding a gun. She hands it to him, and the demon tears out her beating heart, despite her belief that she still has control over Bakwas. To Guy's "respect and dignity" treatment, the demon kills him and inserts the stone into its body.
