When the owner and operator of a luxurious island invites a collection of guests to live out their most elaborate fantasies in relative seclusion, chaos quickly descends.
The enigmatic Mr. Roarke makes the secret dreams of his lucky guests come true at a luxurious but remote tropical resort. But when the fantasies turn into nightmares, the guests have to solve the island's mystery in order to escape with their lives.
The movie opens with an unnamed blonde woman (Portia Doubleday) in a yellow dress running through woods away from a gang of men in black ops uniforms with guns. She breaks out of the tree line and comes across a beautiful big Tiki style mansion set against a beach with a jetty. She breaks into the building and finds an office with a phone, which starts to ring. When she picks up, she hears a man's voice who is calm and reassuring. She tells him she's been kidnapped and taken to an island, and he promises to send help. However, he calls her Mrs Maddison, leading her to ask how he knows her name. The man on the phone simply says "because you are one of my guests". The armed gang then breaks into the office and grabs her, dragging her out of view.
We then cut to what we presume to be the following morning. We see a woman in a white dress walking along the coast line in front of the mansion. She is wearing a name tag saying "Julia". She sees a plane flying in and preparing to land. She runs into the office to tell a well dressed man in a white suit (Michael Peña) that the plane is arriving. There are many people in white all with name badges, which appears to be a staff uniform. The man is clearly in charge, and his voice is the same one that spoke to 'Mrs Maddison' on the phone the previous night.
The plane lands at the jetty and five people climb out. They are Melanie (Lucy Hale); a young blonde whose flirty and feisty. Patrick (Austin Stowell); a tall blond man wearing army tags who seems a little shy and unsure how to navigate Melanie's advances. Gwen (Maggie Q); a tall slender brunette who also seems ill at ease and uncertain about the island. JD (Ryan Hansen); a blond classic loudmouthed all-American jock, and his step-brother Brax (Jimmy O Yang); a shorter Asian man who matches his brother's energy like for like. Other than the brothers, the 5 guests are strangers. They all except Brax have won a competition and their prize is to live out their fantasy on Fantasy Island. JD has brought Brax along with him to share the experience. The well dressed man in the suit introduces himself as Mr Roarke, and welcomes the guests inside. Meanwhile, a wild looking man with a large machete watches the guests arriving from his hiding place in the tree line.
Melanie, Patrick and Gwen are each shown to their separate large and luxurious bedrooms, whilst JD and Brax enjoy drinks at the bar. Melanie sits on her bed and looks sadly at a photo of her with a dark haired man, who appears to be a romantic partner. Patrick gets changed, revealing his dog tag necklace. Gwen is led to her room by the staff member called Julia who confides that she is new and doesn't really understand much about the island. Julia is however certain that the island is magical and tells Gwen it will change her life forever. As she leaves, she gets a nosebleed. Gwen then settles into her room but still appears uncomfortable. She hears a noise in her bathroom, and finds wet foot prints on the floor. She goes to investigate but is startled by a split second image of a man covered in burns in her bathroom mirror. He is gone in the blink of an eye, and the wet footprints are mysteriously gone.
The five guests meet in the bar and continue to socialise. They all seem to share in both their excitement for what's to come, as well as their skepticism about how the experience will work. Mr Rourke appears and the guests ask him for further details. He simply tells them that the island will provide their fantasies, however there is one rule; they must continue in their fantasies until they reach their natural conclusions. Gwen is concerned by this and asks why they wouldn't want to complete their fantasies. Mr Rourke says that fantasies rarely turn out how one would expect. He then ends the conversation, informing them that whilst Melanie, Gwen and Patrick's fantasies will start in the morning, JD and Brax will begin theirs straight away. From this moment on, the group is separated, and scenes cut between each of the fantasies.
Mr Rourke leads the brothers through the jungle. JD's fantasy had been simply to "have it all". After walking a fair distance from the mansion, the brothers arrive at another mansion, however this one is far more modern than the main mansion house, and has a pool. There are hundreds of attractive people in swimwear dancing and drinking and having a party. Mr Rourke tells them it is theirs. The brothers are amazed and get stuck in immediately. JD surrounds himself with beautiful women in bikinis, whilst Brax (who is gay) is led away by muscular guys in speedos smoking pot. The party gets underway and the brothers drink their fill and sleep with the 'models'. However, Brax has a disturbing vision of the burning man earlier seen by Gwen. Similarly, the man disappears suddenly, however Brax dismisses it as a hallucination brought on by smoking weed. Brax and JD eventually tire of raving and decide to explore the mansion further. They come across what appears to be the master bedroom, which has large closets full of fancy clothes. They each get changed into matching shiny suits and then discover a panic room hidden behind one of the closets. They continue to explore and come across an armory downstairs which is filled with an arsenal of weapons, from bazookas to hand grenades. The pair play with the weapons, setting off some of them over the bay, and continue to party.
We then see Gwen waking up in her room the following morning, and being summoned to Mr Rourke's office to begin her fantasy. Mr Rourke is taken aback by Gwen's fantasy, and explains it will be harder than most people's fantasies which mainly revolve around money and sex. Gwen explains that 5 years earlier, a man called Allen who she had been dating proposed to her on their 1 year anniversary. She had declined him, and regretted it ever since. As Rourke pushes her to talk about it further, she reveals that she had felt it was too soon, but now yearns for a family life and desires a child. Gwen cannot see how her fantasy could possibly be simulated, and tries to ask Rourke what lies ahead, but he evades her questions, and leads her to a door with snake carvings on it. She walks in to find herself in the restaurant where Allen had proposed. She turns to see Allen himself sat at a table waiting for her. Gwen is in disbelief and asks Rourke and Allen how they had staged it, but Rourke continues to be evasive, and Allen seems genuinely confused by her questions. Gwen relaxes into her date with Allen and chats to him like old times. He then begins to propose and she tries to tell him he doesn't have to, still believing he has been asked there by Rourke. However, Allen persists and asks her to marry him. Gwen is shocked and cannot continue to protest, and accepts his proposal immediately.
Cutting away from Gwen and Allen, we see Melanie and Patrick who are still waiting to begin their fantasies. They discuss what it is they asked for, and Patrick reveals that whilst he works as a Police Officer, he always wished he had joined the Military, and asked to be a soldier. Melanie tells him her fantasy is to seek revenge on a girl who bullied her in high school. She admits that the fantasy is silly, but explains that she'd been deeply affected by the bullying, and had to see a counsellor who she nicknamed "Dr Torture". Rourke then appears and gives Melanie directions to a lift and tells her to go to the basement, where her fantasy will begin.
Melanie gets into the lift and presses a button with a snake motif on it. She is nervous and watches water dripping from the ceiling of the lift as it judders down. She gets out and finds herself in an underground bunker, with a retro panel of lights and switches. Lights come on and behind the panel of switches is a 2 way mirror. Behind the glass is a woman tied to a medical chair in some kind of torture chamber. Melanie identifies the woman as 'Sloane', the bully who she aims to seek revenge on, and initially believes her to be a very realistic hologram. However, the woman is the same one seen earlier being captured by armed men claiming to have been kidnapped and forcibly taken to the island. Melanie begins pressing buttons on the panel, each of which cause a different type of torture, ranging from mild electric shocks to being doused in cold water. She presses one button and screens light up in the room with Sloane and begin playing a sex tape involving Sloane and a man who isn't Sloane's husband. As Melanie presses another button, the screen shows Sloane's Facebook account, and the sex tape being posted. It then shows a hidden spy camera in Sloane's house and the pair watch Sloane's husband seeing the sex tape. He then calls Sloane's phone, and the phone call comes through to the room. Sloane's husband shouts down the phone at her for betraying him, however Sloane is gagged and unable to speak to him. A door to the rear of the room then opens and a tall masked man in a vintage surgeon's uniform walks in wheeling a trolley of medical sharps. He picks up a scalpel and begins to cut Sloane. At this point, Melanie begins to panic as she realises it isn't a hologram, and Sloane is actually there being tortured. She tries to stop the scene, but the man approaches the glass, pulling down his face mask to reveal his face. His mouth has been stapled closed. Melanie recognises the man's face and calls him 'Dr Torture', her former counsellor. He continues to torture Sloane, so Melanie uses an intercom to give him instructions, seemingly playing along. However, she successfully manages to manipulate Dr Torture into electrocuting himself through a combination of the electric shock button and the water. Melanie breaks the 2 way mirror and frees Sloane. Sloane isn't particularly grateful to Melanie, and fails to fully remember her, initially calling her Meredith. Melanie pretends to be another kidnap victim and the pair escape through a back door leading out to the woods.
Patrick is in a range rover with Rourke who drops him off in the middle of the forest. He hands him a bag with an army uniform and sends him out into the woods. He puts the uniform on and walks alone for a while into the woods before being come upon by the wild looking man with the machete. He gives him an ominous warning that 'people die' on the island, and tells him to leave with him. However, a unit of soldiers then approaches and the wild man scarpers. The soldiers ask Patrick who he is, and he plays along and says he lost his unit in the woods. The soldiers are uncertain and say no other units were there but them. They tie Patrick's hands behind his back and march him at gunpoint back to their base camp. Patrick doesn't particularly like his treatment, and complains that this isn't part of his plan. The soldiers don't understand and ignore his complaints, marching him back to base. As they walk they hear a distant explosion which actually transpires to be the sound of JD and Brax playing with weapons, but Patrick and the soldiers assume it to be distant warfare. Upon reaching base, they tell their leader, Lieutenant Sullivan, about Patrick, but Patrick recognises the Lieutenant immediately and becomes distressed. In return, Sullivan asks Patrick why he is wearing dog tags with his name on them. The unit are then set upon by men with guns and shots are fired by both parties. Patrick is caught by a bullet which grazes his arm. He sees men being shot in front of him and realises the violence isn't merely simulated, he then passes out. When he awakes, he finds himself in the woods again with the remaining members of the unit. Sullivan summons him to talk out of earshot of the other soldiers. He asks Patrick why he has his tags again, and also asks why he has a photo of him with his young son and and drivers license with his face and his son's name and date of birth. Patrick tells him that he is his son, and proves it by stating facts only his son would know. Sullivan asks about the last time Patrick saw him and Patrick explains that his father died on a mission in Venezuela. Sullivan tells Patrick that that's where they're heading in the morning. Sullivan then decides to abandon his unit with Patrick and leave the mission if he is going to die. Patrick is dismayed and tells his father that isn't how it's meant to happen. Patrick tells his father that he died a hero, throwing himself onto a grenade that would otherwise have killed the whole unit. Sullivan dismisses Patrick's ideals of heroism and the two men fight, but ultimately Sullivan agrees to go ahead, as Patrick's presence means things may turn out differently this time.
Melanie and Sloane are in the forest together trying to figure out how to escape the island, when they are suddenly set upon by Dr Torture, who seemingly survived the electrocution after all. He attacks Sloane again, pinning her to a tree, but they are then saved by the wild man, who appears and stabs Dr Torture with his machete. He tells them his name is Damon and that he is a private investigator who was sent to discover the secrets of the island. He explains he was lured in by fantasies of his deceased daughter brought back to life, but that every fantasy becomes twisted. He mentions that his daughter became like a zombie with black water streaming from her eyes. He tells them to follow him, but as they leave, we see Dr Torture's eyes swell and begin to seep black water. Damon leads the women to a cave entrance. Inside, there are a catacomb like maze of tunnels and flooded passageways, which he warns are easy to get lost inside. As they make their way through, they are stalked by both the ghostly image of the burning man, and a doppelganger of Sloane who crawls along the ceiling. Eventually, they make it to the centre of the maze of tunnels and find a large dripping stone suspended above a well of black water. The stone reveals images to whomever stands before it of what they fantasise about. Damon sees his young daughter, Sloane sees her husband, but Melanie sees various images including Sloane tied up in the torture chamber. Sloane realises that Melanie is not another kidnap victim, but a guest, but Melanie explains that she never thought they would actually kidnap Sloane, and the two argue. Damon reprimands them and tells them to focus on getting off the island safely. He gathers water from the well beneath the stone into a bottle, explaining that the water holds the stone's magic, and tells them to take it off the island so the world can understand its magic. The trio then leave the tunnels and begin to head back to the mansion to take the next plane off the island. As they walk, they hear distant gunfire, which transpires to be the fight that broke out between the soldiers and the men in the woods. As they walk along a high cliff edge, the women continue to bicker with one another and Damon reprimands them again, reminiscing that they remind him of his young daughter who he feels he failed to protect. They are then attacked once more by Dr Torture, who now has black eyes streaming with black water and seems to have magically reanimated after being stabbed by Damon's machete previously. Damon fights him, but he is now invincible and unafflicted by Damon's attacks. Damon looks at the women and then launches himself at Dr Torture, throwing both of them off the cliff edge to their deaths at the bottom of the ravine. Melanie and Sloane are shocked at his sacrifice and agree to work together to escape, heading back to the jetty by the mansion.
Cutting back to JD and Brax, their party comes to an abrupt halt as shots are heard outside the house. They aren't sure what is happening but decide to hide in the panic room off the master bedroom. Meanwhile men in clown masks carrying guns make their way into the house. The brothers run upstairs with the models and open the hidden door but Brax stops to use the phone to call Rourke. Brax asks Rourke for help, but Rourke simply says "I'm afraid the problem with having it all is that usually other people want to take it". Despite JD's pleas to leave the phone and hide in the panic room, Brax continues to beg with Rourke, and then the models close the door, locking the brothers out. The masked men reach the bedroom and Brax and JD surrender. The brothers attempt to reason with their captors, explaining they are merely guests on an island and that Rourke is the man in charge. However, the captors don't believe them, and the leader of the group threatens JD with a knife, demanding to know where his money is. JD then lies and tells him it is downstairs, giving directions to the armory. The leader makes Brax show him the way, leaving JD with one of the masked men pointing his gun at him. Brax leads them downstairs, but manages to slip into the armory and close the door before they can follow him. He then shoots one of the men through the door. He then comes out holding a grenade, which he threatens to detonate if they don't surrender their weapons. Brax puts on the mask of the dead man to trick the one left guarding JD and follows the others out of the room still holding the grenade. However, as they leave, the dead man's eyes appear to burst and black water streams out of them.
Gwen wakes up in her room in the mansion and finds a wedding ring on her finger and a photo on her phone of her wedding day to Allen. She walks outside and talks to Rourke who convinces her that her fantasy has become real. He tells her five years have gone by since she married Allen. She turns to see Allen sat on the beach, she then sees a child with him, who Rourke tells her is her daughter. Gwen is shocked and finally realises the island's powers are real. Rourke tells her that he also has a fantasy that is granted by the island. His wife died and the island has reanimated her. So long as he stays on the island, she is still with him. Gwen spends time with Allen and her daughter 'Lila' and plays with them in the sand. However, she is troubled and goes to speak to Rourke. Gwen demands to have a different fantasy. She tells him if she had known the fantasy would be real, she would have used it more responsibly to right a wrong from her past. Rourke initially refuses her, and Gwen is very distressed. As she paces around the mansion, she stumbles across Julia who is very sick and coughing up blood. Gwen helps her and then discusses her problem. Gwen explains that six years ago she accidentally caused a fire in her apartment, killing her neighbour Nick Taylor. Julia comforts her and encourages her to ask Rourke again. She goes back to Rourke's office, and this time successfully convinces him to give her a 'do-over'. She goes through the doors engraved with snakes a second time and this time she finds herself in her old flat as it burns. She can hear Nick screaming for help and runs to his door to help him. However, as she runs she passes JD and Brax in the hallway. She asks them to help her but they refuse. Gwen makes it to Nick's door but is unable to get inside. She then heads down to the street and begs the nearest Police Officer for help, who turns out to be Patrick. He looks at the burning building and also refuses to help, advising she wait for the Fire Department. Gwen goes back in alone and gathers some tools from her flat which is now full of smoke. She manages to break into Nick's flat, but passes out from smoke inhalation before reaching him. She is then dragged out of the burning building and back through the doors into the mansion by none other than Julia. Julia asks whether her fantasy came true, but Gwen admits that she failed to save Nick. This perplexes Gwen, who now knows the power of the island and so cannot understand why her fantasy didn't work. She then suggests to Julia that perhaps someone else's fantasy is taking precedent over hers.
Patrick, his father and the other soldiers head out on their doomed mission to rescue Americans taken hostage. As they approach the house, it becomes apparent that the hostage situation is actually at JD and Brax's mansion. As the unit heads upstairs to the master bedroom, Sullivan tells Patrick to stay outside as he anticipates a second assault. As Patrick waits outside, he sees the masked men leaving a downstairs room being followed by Brax (who he doesn't recognise due to the mask) who is still holding the unlocked grenade and using it to threaten the men into submission. Patrick startles them and threatens Brax who takes off his mask. The two men are confused how the other has appeared in their fantasy, and the masked gang men use the distraction to begin fighting them. The leader wrestles the grenade off Brax. Meanwhile, Sullivan and the soldiers reach the bedroom and kill the masked man who was guarding JD. They free JD and the models in the panic room. As they leave, the dead masked man on the floor begins to seep black water out of every part of his body, he twitches and then gets back up to his feet. When the group come back out, the gang leader goes to throw the grenade at the soldiers and JD, which appears to be the moment when Patrick's father sacrificed himself to save his unit. However, this time Patrick is there and manages to shoot him before he can throw the grenade. Brax catches the grenade and replaces the pin. The group celebrate their victory, with JD and Brax relieved to have survived, and Patrick and his father relieved that they avoided the inevitable grenade. However, the celebration is short lived, as the reanimated corpse of the masked man then appears and opens fire on the group, hitting JD square in the back. The soldiers fire back, but this time, their shots don't appear to do more than temporarily stun him. The other masked men begin to reanimate and shoot the group, killing several soldiers, who realise they have no option but to retreat. Brax is stunned by JD's death but Patrick drags him away as the group flee. Sullivan is hit, and realises he cannot run, so stays to continue shooting, allowing Patrick and Brax to escape as the only survivors.
Melanie and Sloane make it back to the mansion where they meet up with Patrick and Brax. The group try to head to the jetty, but are stopped by Rourke and his staff who restrain them. Gwen then appears and reveals what she suspects has actually happened; she explains that Nick Taylor was a young man who died and that she believes they are all in some way connected, and have been brought to the island to fulfill someone else's fantasy to avenge his death. Patrick admits to being a coward that night, Brax admits that he and JD had shared a flat with Nick, and not bothered to disturb him when the fire alarm sounded. Melanie revealed that she had been meant to date Nick that night, but bailed on him at the last minute due to her insecurities which were caused by Sloane's bullying. Melanie concludes that if she hadn't have bailed, Nick would never have been in the apartment building at the time of the fire. At this moment, they hear the plane arriving, and Patrick fights the staff and the group get out to the jetty. However, the plane is shot down by the group of reanimated masked men who then open fire on the group, who flee to the forest.
The group (now consisting of Gwen, Melanie, Sloane, Patrick and Brax) make it to a clearing in the forest and stop to catch their breath. They argue over what to do, and Gwen again theorises about who could be behind the fantasy, as it cannot be Nick as he died. She recalls that Julia pulled her out of the fire, pointing out that none of the staff would ever interfere with a fantasy like that. She then suggests that it could be Julia's fantasy after all, and wonders whether Julia is a relation of Nick's such as his mother. Melanie is quick to support this theory. The group continue to argue with regards to the best course of action, with Melanie then suggesting they destroy the island's powers by destroying the magic stone in the cave. Sloane tells the group about the stone and explains it is the source of the magic water. Brax points out that they all drank the water which is likely how their fantasies became granted. Patrick still has Brax's grenade and suggests they use it to destroy the stone. The group agree and make their way to the cave entrance.
Once inside the catacombs, Melanie warns them to stick together, but they each get lost in the maze. After Gwen and Brax get lost, Melanie asks Patrick for the grenade and offers to go ahead and destroy the stone with Sloane but Patrick refuses. Once they have all separated, they each encounter their own horrors. Brax is lured into a tunnel by a vision of JD. However, when he reaches him, JD rejects Brax and torments him by playing on his familial insecurities. He then begins to seep black water from his eyes and collapse as one of the reanimated masked gunmen appears and chases Brax. Sloane is met by the doppelganger of herself which stalked her earlier. She tells her to face up to the cruel person she truly is. They fight, before Sloane is then grabbed by the reanimated Dr Torture who has also appeared. Gwen stumbles across her daughter Lila who is bashing in Allen's head with a rock. She accuses her mother of being undeserving of her and introduces her 'friend' who is the burnt man. He then morphs before Gwen's eyes to reveal he was Nick, the same man in the photo on Melanie's phone. Patrick is wading through the flooded tunnel when he is attacked by his reanimated father, who also now has black streaming eyes. Patrick fights his father, eventually killing him by holding him under the water until he vanishes. Gwen speaks to the ghost of Nick and begs him to end the fantasy. He reminds her that he isn't behind it, and Gwen cries out begging Julia to end it. Nick however appears confused, and asks "Who's Julia?". As Patrick walks away from the pool where he killed his father, he is then stabbed by none other than Melanie who then steals the grenade.
Brax and Gwen find one another, and Gwen reveals that Julia isn't the one behind the fantasy. Melanie then appears, followed by Dr Torture who is holding Sloane. Melanie then reveals that the revenge fantasy was hers all along. She explains that she did in fact date Nick and that she had fallen in love with him. When he died, she was disgusted that the others appeared to move on with their lives, whilst her future was stolen. She discovered the island and plotted the whole thing, making her fantasy before the others. They question why she has allowed them to survive this long, and Melanie explains that she wanted them to survive to see what she'd done, harking back to an earlier comment she'd made about there being no joy in a revenge if the victims don't find out why. Sloane then goads Melanie, telling her she isn't impressed by her scheme, and calls her pathetic. Melanie becomes enraged and whilst her back is turned, Gwen and Brax grab Melanie's knife and the grenade. Gwen punches Melanie, briefly stunning her, and they stab Dr Torture, causing him to release his grip on Sloane.
They run and manage to make it to the room in the centre of the catacombs with the magic stone. Gwen approaches, holding the grenade and preparing to throw it, but is halted by the vision of Lila reflected to her in the stone. The vision then changes to Julia, and she realises that Rourke had appeared behind her. It is revealed that Julia is actually Rourke's fantasy, as she is his wife. However, as Rourke had wished for her just as she had been, she is very sick, and destined to continuously worsen and die, then reappear with no memory of Rourke or how she came to be there. Rourke tired of explaining to her repeatedly that she was his wife, so began telling her she was his PA. Whilst Gwen is distracted, Rourke grabs the grenade out of her hand. Melanie then appears flanked by reanimated masked men and Dr Torture. Gwen begs Rourke to end Melanie's fantasy, but Melanie points out that if he doesn't then he will lose Julia. Gwen pleads with Rourke and questions whether watching Julia suffer and die on repeat is worth holding onto. Rourke maintains he cannot give her up, and throws the grenade (still pinned) into the depths of the well. Melanie's reanimated zombies then grab Gwen, Sloane and Brax, and Rourke retreats. However, as Rourke wanders through the tunnels, he meets Julia who is now very sick and dying. Julia then begs him to save the guests, and manages to convince him to let her go.
Rourke then re-enters the central chamber and interrupts Melanie as she gloats. Patrick also limps into the room, appearing from another tunnel clutching the stab wounds to his abdomen. Rourke points out that all of his guests get a fantasy, and attention turns to Sloane, who hadn't had one. She still had the canister of water collecting earlier from the well by Damon. She drinks the water, and uses her fantasy to wish for Melanie's death. The burnt ghost of Nick erupts from the well and grabs Melanie, dragging her back into the well and under the water with him. The reanimated masked men and Dr Torture dissipate into black water as Melanie drowns. However, as Melanie is drowning, she finds the grenade. In a last ditch effort to exact her revenge, she bursts up out of the water and throws the grenade out onto the floor, before being pulled back under by Nick. Patrick sees the grenade and hurls himself onto it, absorbing the blast and killing himself to save the others.
Gwen, Sloane and Brax then wake up in the mansion and meet in the bar where Rourke informs them that Patrick died a hero, just as his father did. The trio head out to board the plane to leave. Gwen asks Rourke about his wife, and he vows to stay on the island as its protector and live the remainder of his life without regret. As they board the plane, Brax is stood at the end of the jetty gazing over the water. Rourke tells him that as he partook in his brother's fantasy, he never had a wish of his own fulfilled. Brax wishes that JD can return home and live, but Rourke warns him that the fantasy cannot be sustained if the wisher leaves the island. Gwen and Sloane take their seats on the plane and reflect on the events. The plane then begins to take off, and they call out to the pilot, warning him that Brax hasn't yet boarded. However, JD then leans around a divider wall and tells them he doesn't think Brax is coming. The plane takes off and JD leans out the window to wave goodbye to his brother. Brax is thrilled to see his brother alive again, and seems happy with his choice. He and Rourke discuss what he will do now he's stuck on the island, and Rourke tells Brax he'll need a uniform.
It is never revealed precisely what role Rourke plays in the fantasies, or why he chooses to stay once his wife is gone. The film closes with the island and its staff, now including Brax, still fully operational, leaving the movie open to potential sequels.
