Everly (Salma Hayek), a prostitute working for the brutal criminal overlord Taiko (Hiroyuki Watanabe), is being attacked in her apartment by Taiko's enforcers after he discovers that she has been working with the police to bring down his organization.Everly manages to escape into her bathroom and use a cell phone (hidden in the flush tank of the toilet) to call Detective Robertson. She conveys the message that Taiko knows about her betrayal against him and his organization. She is expecting help, but Robertson lets her call go to voicemail, indicating that he does not want to get involved. Everly knows that she is alone.
However, though Taiko expected his men to kill her easily after torturing her, Everly retrieves a gun she had hidden (in the same toilet flush tank) and manages to kill all of her attackers. Everly is shot in the belly during the attack.Taiko then calls Everly on the cell phone of one his henchmen and reveals to her that he knew about Robertson and had already beheaded him. Taiko then says that he knows the location of his mother and daughter and has dispatched his men to intercept them. Taiko promises to turn Everly's daughter Maisey into a prostitute for some of his older clientele.Taiko then begins a sadistic game of toying with Everly, dispatching numerous hired killers and offering the other prostitutes (like Zelda (Caroline Chikezie), Dena (Jennifer Blanc), Anna (Gabriella Wright), Elyse (Jelena Gavrilovic) and Lizzy (Dragana Atlija)) in the building (which he presumably owns) a bounty of $50,000 if they manage to kill her.Everly manages to kill Zelda and Lizzy. Elyse and Dena shoot each other while fighting over who gets to kill Everly. Everly then convinces Anna to let her go with the promise that Anna will get the chance to kill her later that same night.
Meanwhile, using the cell phone Everly attempts to contact her mother Edith (Laura Cepeda) and young daughter Maisey (Aisha Ayamah) to save them from other henchmen of Taiko and get them out of the situation alive. But her mother is being stubborn and refuses to leave without her. Everly asks Edith to meet her at McCoy Park downtown. Everly has a stash of cash hidden underneath the floorboards of her apartment and wants to get the cash to her mother. This way Edith and Maisey can disappear, out of Taiko's reach.Dead Man (Akie Kotabe) is the only survivor from the shootout inside Everly's apartment, but he is shot in the intestines and will eventually bleed to death.
It is revealed that Everly was a prisoner in the plush apartment after having been kidnapped by Taiko four years earlier and forced into prostitution. She has had no contact with Edith or Maisey during that time, and they are confused and angry at the deadly situation they are suddenly thrust into.Everly cannot get out of her apartment building as it is now guarded by corrupt cops working for Taiko. They won't kill Everly, but simply put her back in her apartment. Everly calls her mother and tells her to come to come to her apartment building, as Everly is unable to get out. To hide the massacre from Edith and Maisey, Everly hides all the dead bodies in her apartment and cleans up all the bloodstains.Everly tells the Dead Man that it was the police who reached out to her when she called for a plumber to fix her toilet. The police promised her an escape and a new identity in the witness protection program.
Edith and Maisey manage to get inside the apartment building by telling the guards downstairs that she wants to visit an apartment, different to Everly's. Everly is attacked by another one of Taiko's men. Everly explains the situation to her mother and wants her to take the money and leave. The Dead Man finally dies from his injuries. Everly knows that she cannot escape her fate at the hands of Taiko. Just then she and her family attacked by a killer dog released by Taiko's henchman. Everly manages to kill the dog by making it chase a grenade.Everly realizes that the attacks are not going to end and so she forces Edith & Maisey to take shelter in the apartment of a neighboring prostitute Anna, who has made temporary truce with Everly in the promise of a max payout later.
Everly is caught by The Sadist (Togo Igawa) and some of Taiko's henchmen and tied up in a room. He has her tied up and placed in a cage while he taunts and tortures her with battery and sulfuric acid, along with other corrosive liquids, at one point pouring water into her eyes while she screams in fear, believing it is acid. When The Sadist pours some of the corrosive liquid onto a rope binding her wrist, the rope starts disintegrating.
During Everly's torture, Edith bursts in with a gun and kills 2 of The Sadist's torturers but is subdued by The Sadist while he performs the same torture as he did with Everly on her. Everly desperately struggles to escape and help her mother. She breaks one of the ropes that The Sadist had loosened before and uses her free hand to kill one of the assailants with a hidden gun.When The Sadist realizes she has a hidden gun, she shoots him twice in the lung. The last torturer runs toward Everly with a weapon, only to die when she shoots him in the chest and neck. She escapes her remaining restraints and goes to help Edith. The Sadist stands up (being still alive) and knocks Everly down and is about to kill Edith when Everly forces a poisonous liquid down his throat, melting his intestines out of his stomach which kills him.
As Everly fights off the numerous attacks, the foes become more outlandish; originally, they are simply armed thugs, but eventually they appear in costumes and with themed groups for extravagant tortures.
Eventually, Edith is killed by a sniper hired by Taiko from the building across from the apartment on the orders of Taiko to punish Everly. When the sniper shoots a rocket-propelled grenade into the apartment, Taiko beheads him in anger, believing that he killed Everly, though she survives. Everly is finally subdued but manages to kill Taiko after his soldiers have left by stabbing him with his own sword.One of the Sadist's compatriots, the Masochist (Masashi Fujimoto), attempts to slash Maisey before Everly kills him with a single head-shot.
Afterward, Everly reconciles with Maisey, who was under the protection of one of Everly's enslaved neighbors (who was eventually killed). Everly seemingly succumbs to her wounds, but with Maisey still alive and potentially safe with the death of Taiko.However, immediately before the credits, there is audio of a beeping heart-monitor and gasp of breath to indicate that Everly has not died after all.
