After two years Mario lost his gangster empire, he arrives to his old hometown where everything started many years ago with many other gangster he also consider as family. People he have not seen in a very long time. With no knowledge if they still are on the run from the Bøtteføtte and the government or taken by the government or worse, the new gangsters, Mario takes a big risk returning home with gangsters of Bøtteføtte everywhere and the police looking in every street to find the remaining gangsters including himself. Mario has to blend in and try to come through the city without being seen or worse captured by the new gangsters or the government.
Two years earlier Mario lost is empire and many of his family is taken by the government or worse the new gangster who has the power. About two whole years, Mario has been on the run from everyone who is after him and haven't talk to no one of the other gangster who is also on the run. Mario decides to return to his hometown, where everything started many years ago before City of Gangsters. Mario tries to find remaining gangster who he haven't talk to in many years. Mario try to blend in with other clothes and finds applies so he can manage through this nightmare. While he tries to go through the city, one of Bøtteføttes closest loyal gangster Ms. Saphina and her gangsters have taken two of the gangsters who worked for Mario. Before Mario are through the city and arrived in safe haven, he finds them and spying on them and find out two of his gangsters who is still loyal are taken and Ms. Saphina who is very dangerous try to find Mario herself with her crew and he is shortly spotted and have to run through town to shake those gangsters off, so he can go on and find the other who is still on the run and find a safe haven. Before that happened, Mario returns to his old apartment where he meets Mia. One of the gangster who was with him from the start, that time he started City of Gangsters with also his half brother, who he doesn't know where is. Mario is paranoid, he don't know if he can trust Mia or others because of the gangsters who worked for him have betrayed him, but find out quick that Mia is still loyal and have always been and will be loyal forever. Mia is one of the closest, Mario can call family and Mia know exactly how to make Mario calm down and can always convince him to trust the right people including herself. They talk for a while and fill each other in on what happened the last two years and the decides what to do next and they leave the apartment together.A whole year after this Mario meets his half-brother Michael. Michael tell Mario where he has been go have a new plan for City of Gangsters. A New Game is about to start and Michael tell Mario what to do next.
