On January 28, 1986 the Shuttle Challenger broke apart just 73 seconds after launch and NASA lost seven astronauts. Investigations were carried out to discover what exactly went wrong during the launch, and in the preparations for the day that led to such a catastrophic failure, and resulted in the eventual termination of the Shuttle program.

The Concorde epitomized elegance and aviation luxury, crossing the Atlantic in about three hours, at twice the speed of sound, between 1976 and 2003. On July 25, 2000, Air France Flight 4590 crashed shortly after take off from Charles de Gaulle airport, in Paris, killing all 109 people aboard. In a long and exhaustive investigation, the reasons for the crash were finally discovered.
