She began her career in 1927 in Ljubljana and later became the principal soprano at the Croatian National Theatre. Milanov made her debut at the Metropolitan Opera in 1937 and performed there until 1966. Besides her opera work, she was known for her skills as a concert singer, vocal coach, and teacher.
